Clients, please fill out this form to ensure we take proper care of your pet each and every time they are in our boarding facility. We need the following information: Pet's name(s), how long they will be with us, any medications they need while here and how often to give it, and a list of items that you have brought with them. Please refrain from sending dog beds, as blankets, toys, etc. As most patients tend to chew through them during the length of their stay. Understand that any item on your dog other than their collar will be removed upon arrival; this includes harnesses, sweaters, and collars that do not fit properly or have large objects. Please keep in mind items you leave may not go home in the same condition they arrive.
